There are various ways to do it, but most do not use "raw" water. Some sort of a heat exchange device is between the "raw" ocean or lake water. This may be in the form of a keel cooler or "raw" water going through heat exchangers. Besides keeping mud and trash out of the engines, this also allows temperature regulating valves to maintain a constant temperature on the engine for maximum performance and efficiency. A little like the thermostat on a car engine.
