This differs depending on the cargo and type of dock (ie Shiploader or Gravity Dock (chutes)). But for purposes of this example lets assume loading ore at a shiploader, like Two Harbors or DM&IR. Each shuttle on the shiploader can be turned on or off individually as well as extended or retracted. So the mate in the pilothouse is in communication with the shiploader at all times. And by varying the location of shuttles, and how quickly and from what tanks they pump out the ballast, they do their best to keep the ship level. Some mates deliberately list a ship in the beginning in anticipation for the cargo to shift as the piles become higher in the hold. I hope this helps clear it up for you.
