Dave, even in this day of wonderful technological achievement it still comes down to one person making the right decision at the right time after interpreting the information presented to him. Generally an accident like this results in a series of things going wrong, often refered to as an "accident chain" by the pros. Take one link out of that chain, and the incident never happens. My rule when underway in open water has always been" one mile in good vis, two miles in bad vis". If I can't maintain that separation I take steps to prepare for any potentiality that I can think of to avoid mishap. Something like this should never happen, but it does and that is why we try to keep qualified, capable people command.
